Anxiety/OCD:
Indications for: Doxepin HCl Concentrate
Anxiety.
Adult Dosage:
Soln: dilute with 4oz juice, water, or milk. Mild: 25–50mg/day. Mild-to-moderate: initially 75mg/day; range 75–150mg/day. Severe: max 300mg/day. May give as single (up to 150mg) or divided doses; 150mg caps for maintenance only.
Children Dosage:
Not recommended.
Doxepin HCl Concentrate Contraindications:
Urinary retention. Glaucoma.
Doxepin HCl Concentrate Warnings/Precautions:
Increased risk of suicidal thinking and behavior in children, adolescents, and young adults; monitor for clinical worsening and unusual changes. Screen for bipolar disorder. Cardiovascular disease. Angle-closure glaucoma. Seizures. Psychosis. Poor metabolizers (those with reduced CYP2D6 activity). Avoid abrupt cessation. Write ℞ for smallest practical amount. Elderly. Pregnancy. Nursing mothers.

Doxepin HCl Concentrate Interactions:
Hyperpyretic crisis, convulsions, death with MAOIs; discontinue at least 14 days prior to initiating doxepin. Potentiated by CYP2D6 inhibitors (eg, quinidine, cimetidine, phenothiazines, SSRIs, Type IC antiarrhythmics); may need dose reduction. Do not initiate within 5 weeks of discontinuing fluoxetine. Avoid alcohol. May block guanethidine.
Adverse Reactions:
CNS effects (eg, drowsiness, overstimulation, extrapyramidal symptoms), anticholinergic effects, hypotension, GI upset, photosensitivity, endocrine effects.
